We know insurance and payment can feel confusing, especially when you're already dealing with pain or an injury. At Functional Living Solutions Occupational Therapy, we want insurance and payment questions to be as simple as possible. Coverage can vary from one insurance plan to another, so we are happy to help you understand your benefits before your first visit.

Medicare

In-Network

We are in-network for occupational therapy services covered under Medicare Part B.

Blue Cross Blue Shield of Michigan

In-Network

We are in-network when services are covered under your individual plan.

Blue Care Network (BCN)

Out-of-Network

Some BCN plans include out-of-network benefits, while others may only cover in-network care. We can help check your benefits before you schedule.

 

If your BCN plan includes out-of-network occupational therapy benefits, we can discuss your payment andclaim options with you.

Priority Health

Out-of-Network

Some plans may include out-of-network benefits, require prior authorization, or may not cover routine out-of-network occupational therapy. We can help check your benefits before you schedule and explain what options may be available to you.

Other Insurance Plans

Have a different plan? Give us a call. Many insurance plans include out-of-network benefits that can help cover your services, and we're happy to help you find out what yours offers.

Workers' Compensation

We accept Workers' Compensation claims. Coverage varies by claim, so before scheduling, please have the following information ready:

  • Workers' Compensation insurance carrier
  • Claim number
  • Date of injury
  • Adjuster or claims representative name and phone number
  • Employer information
  • Attorney information, if applicable

Auto Insurance

We accept auto injury claims. Coverage varies by claim, so before scheduling, please have the following information ready:

  • Auto insurance carrier
  • Claim number
  • Date of accident
  • Adjuster or claims representative name and phone number
  • Attorney information, if applicable

Private Pay

Private-pay options are available for patients without insurance coverage, or who simply choose not to use insurance for their care. We're happy to talk through what that looks like for your situation.
